The Structured Family Caregiving program (SFC) offers financial and support services to Georgia families that deliver care at home for a senior or disabled person. You may meet SFC requirements and be eligible for financial compensation if:

    • The person you take care of is Georgia Medicaid-eligible (contact us if you are unsure about eligibility)
    • The person you take care of needs assistance with daily living activities such as feeding, dressing, bathing, transferring, transportation, meal preparation, and more.
    • You live with the person as the primary caregiver
    • You are related biologically as a daughter, grandson, etc. or are related through marriage as a daughter-in-law, son-in-law, etc.

CCSP / SOURCE

Medicaid Waiver – Community Care Services Program Waiver (CCSP) /Service Options Using Resources in a Community Environment (SOURCE )

CCSP is a Medicaid waiver program that provides a range of community-based services to assist individuals who are older and/or have disabilities to achieve safe, self-reliant lives. SOURCE is an enhanced primary care case management program serving frail elderly and disabled beneficiaries. The program works to improve the health outcomes of persons with chronic health conditions by integrating primary medical care with home care services through case managers..

Nexgen Home Care Inc is a Georgia-approved, participating provider of at-home care services to adults eligible to receive the CCSP/SOURCE waiver. This means persons who qualify for the CCSP/SOURCE program may receive homecare  services at no charge or a small monthly cost-share as Nexgen Home Care Inc is reimbursed directly by the State of Georgia.

Common medical conditions include:

Asthma or other pulmonary problems

    • Diabetes
    • Arthritis (rheumatoid and osteoporosis)
    • Stroke
    • High Blood Pressure
    • Amputations
    • Alzheimer’s Disease
    • Spinal Cord Injury
    • Dementhia
    • Other medical conditions that causes a need for assistance with one of the Activities of Daily Living (ADLs)

Veterans

If you are a veteran, spouse of a veteran, or surviving spouse of a veteran, you might qualify for home care services through the Veteran’s Administration (VA) Aid & Attendance program. Nexgen Home Care Inc is able to pre-qualify veterans for benefit eligibility and streamline the application process for these benefits so that our services can be provided to you at no out-of-pocket cost. You can receive up to $2200 a month for these covered services.

Long Term Care / Workers Compensation

We partner with many long term care (LTC) and workers compensation (WC) carriers to provide you the best in home care services possible. If you have LTC insurance or WC insurance, we will work with them to set up the care plan suitable for your needs.

Private Pay

Compared to expensive assisted living centers or nursing homes with fancy furniture and high fixed costs, what you get from us is experienced, one-on-one care to help seniors, adults and children live in the familiar surroundings of home. Our compassionate caregiver services can help you avoid the stress and headaches of moving your special person to an assisted living facility where receiving attention can be a challenge, even at a far higher price!
